Our service coordinators work with you and your family to develop a service plan customized to your unique needs.
We fulfill the role of apprising you of resources available to you; services that are specific to improving your or your loved one’s life.
As your advocate, your Service Coordinator can help you access the following services:
- Community Resources
- Housing Services
- Medicaid Benefits
- Medical Services
- Vocational/Educational Resources
- Waiver Services, etc.
Your Service Coordinator also works collaboratively with your team with the ultimate goal of helping you gain or maintain a level of independence!
